Re-Entry and Transition Services

Planning for transition from Adobe Mountain School to the community begins at the time the youth's individualized Continuous Case Plan (CCP) is first developed. When placed on conditional liberty, the youth’s treatment and programs are extended into the community. Parole status consists of community supervision and community services. Adobe Mountain School staff work closely with Community Corrections staff to ensure that upon release to parole the treatment, programming, health, and education needs of the youth will continue to be met. Arizona Department of Juvenile Corrections (ADJC) Education Transition Specialists provide youth with transitional education services and help the youth reintegrate into an educational system that best fits their individual needs and strengths. Family Services Coordinators assist in arranging services and treatment, including the coordination of individual, group, and family counseling. They also provide ties with existing community social services. This increases the number of youth who can be successfully placed at home, rather than in out-of-home placement, and links families with services. ADJC also partners with public and private entities to provide a variety of additional transitional services to youth in the community. Some of these partners include:Goodwill of Southern Arizona, Maricopa County Education Service Agency, Regional Behavioral Health Authority, Tribal Regional Behavioral Health Authority, and YPIC Workforce Development.
